JH/HS Winter Sports Parent Meeting

We are so excited to Kick Off our Winter Sports in just a few weeks! If your child is planning on playing a winter sport, please plan on attending the meeting on Tuesday, October 29th. In order not to duplicate forms, if your child played HS Soccer (Boys or Girls), JH or HS Cross Country or JH or HS Volleyball, these are the forms that you will need to fill out:

If your child did NOT play a Fall Sport and has a current physical, please fill out the forms ABOVE plus the following:

Avita Consent to Treat

If your child is in need of a physical you, your child and your physician will need to fill out the following forms:

Physical/Concussion Form

Lisa Parrott will be collecting all forms and payments for Athletic Fees and/or Athletic Passes at the meeting on the 29th.

Athletic Fee:

7th-8th:  $75

9th-12th: $115

*Family Max is $285*

Athletic Passes:

Adult: $75

Family: $175

*Please Make Checks Payable to MCS*

*If you are planning on transporting any of the players to the game the Secondary Office will need a current copy of your car insurance as well as the following forms:
